ABSTRACT

Objective:To assess the predictive value of 18F-fluorodeoxyglucose (FDG) PET/CT imaging and relevant factors in the prognosis of patients with classic Hodgkin lymphoma (cHL) before or after autologous stem cell transplantation (ASCT). Methods:From January 2008 to June 2017, 55 cHL patients (28 males, 27 females; age: (28.8±9.6) years) confirmed by pathology in Shanghai General Hospital were retrospectively included. 18F-FDG PET/CT imaging was performed before ASCT in 43 cases and after ASCT in 34 cases (22 patients underwent the imaging both before and after ASCT). Patients were divided into positive group (≥4) and negative group (<4) according to 18F-FDG PET/CT imaging results using Deauville 5-point scale. The predictive value of relevant factors in the prognosis was evaluated with progression-free survival (PFS) and overall survival (OS) using Kaplan-Meier survival analysis and log-rank test. Hazard ratio ( HR) was calculated by Cox regression model. Results:Of 55 cHL patients, 29 (53%) had a progression of disease after a median follow-up of 8 months, and 11 (20%) patients died after a median follow-up of 29.5 months, with the 3-year PFS rate of 46.4% and OS rate of 84.5%. Significant differences of PFS rate were found between patients with or without B symptoms, between patients with or without large mediastinal mass, between patients with international prognostic score (IPS) of 0-2 and those with IPS of 3-7, among patients with different effect of salvage chemotherapy (complete remission (CR), partial remission (PR) + stable disease (SD), progressive disease (PD)), and between patients with negative or positive PET/CT imaging results before or after ASCT ( χ2 values: 5.52-20.01, HR: 2.21(95% CI: 1.56-3.12)-5.51(95% CI: 1.86-16.33), all P<0.05). B symptoms and large mediastinal mass were also prognostic factors for OS rate ( HR: 5.28(95% CI: 1.14-24.51) and 4.27(95% CI: 1.24-14.79), both P<0.05). The combination of 18F-FDG PET/CT imaging before and after ASCT was statistically significant for predicting PFS ( χ2=11.28, P<0.01). Multivariate survival analysis showed that the risk of progression in patients with positive PET/CT results after ASCT was significantly higher than those with negative results ( HR=6.20, P<0.01), and the risk of death in patients with B symptoms was significantly higher than those without B symptoms ( HR=5.28, P<0.05). Conclusion:18F-FDG PET/CT imaging results after ASCT have important values for predicting PFS in cHL patients after ASCT, and B symptoms can be used as an important prognostic indicator of OS after ASCT.

Objective@#To investigate the current status of picky eating among children and family-related factors, to provide scientific basis for diet intervention for children.@*Methods@#A total of 1 829 children were selected from 5 kindergartens in Shenzhen and investigated by electronic questionnaire survey from March 18 th-20 th in 2020.The data was analyzed by SPSS 21.0.@*Results@#The reported rate of picky eating behavior among children was 43.8%. Parents-picky eating behavior, children s preference to eating snacks, parental worries on children s appetite, parents attitude toward the food disliked by children were positively correlated to children s picky eating behaviors[OR(95%CI)=3.86(2.74-5.43), 1.44(1.15-1.80), 3.21(2.45-4.22), 2.21(1.65-2.95)]; however, eating various food under the instruction of their parents was negatively correlated to children s behaviors[OR(95%CI)=0.38(0.29-0.49)](P<0.05).@*Conclusion@#The reported rate of picky eating behaviors of children on holiday is lower than usual, but parents should to help them develop good dietary habits, prevent picky eating among children.

ABSTRACT

Objective To assess the value of 18F-fluorodeoxyglucose (FDG) PET/CT in the staging,interim therapeutic and prognostic evaluation of mucosa-associated lymphoid tissue (MALT) lymphoma.Methods Thirty-six MALT lymphoma patients (20 males,16 females;average age:61.7 years) confirmed by pathology from January 2008 to January 2018 were retrospectively analyzed.18F-FDG PET/CT were performed before chemotherapy and radiotherapy for staging.The detective sensitivity was evaluated.The staging results of gastric MALT lymphoma and extragastric MALT lymphoma by PET/CT were compared with Fisher exact probability method.PET/CT was performed in 17 of 36 patients after 4 courses of chemotherapy,and 17 patients were divided into positive group (≥≥4) and negative group (<4) according to scores of Deauville 5-point scale.The progression-free survival (PFS) was evaluated using Kaplan-Meier survival analysis.Results FDG-positive lesions were found in 31 of 36 patients with the sensitivity of 86.1% (31/36).The results of PET/CT were negative in stage Ⅰ patients.In stage]Ⅱ-Ⅳ patients,the results of 18F-FDG PET/CT combined with bone marrow biopsy were in accordance with the results of clinical staging.The accuracy of PET/CT in staging of gastric MALT lymphoma patients was 9/17,which was significantly lower than that of extragastric MALT lymphoma patients (17/19;P=0.025).The PFS of negative group evaluated by interim PET/CT was longer than that of positive group (x2 =4.16,P<0.05).The 2-year PFS rates of the 2 groups were (85.7± 13.2)% and (27.8 ±21.3)%,respectively.The PFS of patients with low expression of Ki-67 was significantly longer than that of patients with high Ki-67 expression (x2=4.22,P<0.05).Conclusions In stage]Ⅱ-Ⅳ MALT patients,18F-FDG PET/CT combined with bone marrow biopsy can improve the staging accuracy.The staging accuracy of PET/CT in extragastric MALT lymphoma is significantly higher than that of gastric MALT lymphoma.PET/CT and Ki-67 can provide effective information on the prognostic evaluation for patients with MALT lymphoma.

ABSTRACT

Objective To investigate the prognostic value of △SUV and Deauville 5-point scoring (5-PS) in patients with DLBCL.Methods Thirty-nine patients (20 males,19 females;median 58 (23-85) years) with pathologically confirmed DLBCL were retrospectively analyzed from January 2009 to April 2015.PET/CT imaging was performed before and after 4 courses of chemotherapy.The optimum cutoff values of △SUVmax and its decline proportion (△SUVmax%) were calculated by ROC curve,and then the Kaplan-Meier analysis and Cox regression analysis were performed in patients with different △SUVmax and △SUVmax%.Patients were also evaluated by the observers using Deauville 5-PS,Kaplan-Meier analysis and Cox regression analysis were performed in patients with scores ≥ 4 and those with scores < 4.x2 test and Spearman correlation analysis were used.Results (1) There were 10 patients (25.64%,10/39) with progressed disease within the 2 year-follow-up.The △SUVmax,△SUVmax% of progressed group were markedly lower than those of non-progressed group:9.55± 11.90 vs 15.61±7.86,71.66% (33.90%-78.91%) vs 87.83%(76.51%-92.43%);t=-2.37,z=-3.25,both P<0.05.(2)The optimum cutoff values of the △SUVmax,△SUVmax% were 11.2 and 72.88% respectively.(3) The patients with △SUVmax < 11.2,△SUVmax% <72.88% or Deauville 5-PS≥4 showed shorter PFS and △SUVmax% was proved to be an independent prognostic factor(x2 =5.734,14.821,5.851,all P<0.05).(4) △SUVmax% and Deauville 5-PS were correlated (rs =-0.633,P<0.001).Conclusions △SUV and Deauville 5-PS could be used for prognosis prediction in DLBCL patients and △SUVmax% might be an independent predictive factor.The values of △SUVmax % and Deauville present a negative relationship.

ABSTRACT

Objective To investigate the value of 18F-FDG PET/CT for the detection of primary malignancy in patients with cervical lymph node metastases from unknown origin.Methods Seventy-eight patients (48 males,30 females,average age (56.4± 14.7) years) who were diagnosed as cervical lymph node metastases from unknown origin by fine needle aspiration underwent 18F-FDG PET/CT from 2007 to 2012.ROI was drawn with SUVm~ calculated.The distribution of the lymph nodes was classified according to the standard levels Ⅰ to Ⅵ,as well as the retropharyngeal and supraclavicular stations.The short axis of lymph node was defined as the lesion size.The final diagnosis was confirmed by pathology and clinical follow-up.Data collection and interpretation included identification of primary malignancy,pathologic types of primary tumors,distribution,size and SUVmax of the metastatic lymph nodes.One-way analysis of variance and linear correlation analysis were used for data analysis.Results In 78 subjects,75 were diagnosed as having malignant and 3 as benign lymph nodes by excisional biopsy.18F-FDG PET/CT detected primary malignancy in 56 cases (71.8%,56/78).In these 56 cases,57.1%(32/56) had head and neck cancer with metastatic lymph nodes mainly distributed in levels Ⅱ,Ⅲ and Ⅳ (90.6%,29/32).The remaining 24/56 (42.9%) patients had primary malignancy of the trunk with metastatic lymph nodes primarily involving supraclavicular fossa (95.8%,23/24).There was no statistical difference in size and SUVmax of lymph node among different primary locations or pathological types (F=0.037-2.413,all P>0.05).Conclusion 18FFDG PET/CT may play an important role in the detection of primary cancers for patients with metastatic cervical lymph nodes from unknown origin.

ABSTRACT

Objective To introduce a new classification system for femoral intertrochanteric fractures and propose classification to assist surgical management.Methods A total of 176 cases of femoral intertrochanteric fractures treated from September 2007 to October 2011 were enrolled in the study.X-ray and three-dimensional spiral CT reconstruction were performed to define fracture shape,location,number of major fragments and displaced direction.As combined with the intraoperative findings and cadaveric anatomical study,the femoral intertrochateric fractures was simulated using a three-dimensional entity of normal proximal femur produced by Mimics 11.1 program.A new classification system for femoral intertrochateric fractures was proposed.Results The new system categorized intertrochateric fractures of the femur into five types (Type Ⅰ,Type Ⅱ,Type Ⅲ,Type Ⅳ and Type Ⅴ) based on the integrity or destruction of the medial column,lateral column and posterior column set and divided on the basis of bone connection of femoral head-neck fracture fragments with the bone in intertrochanteric region.Conclusions The new classification system with introduction of three-column concept is brief and practical and has a clear definition of fracture stability.Besides,the new classification-based treatment is recommended.

ABSTRACT

Objective To explore the clinical diagnostic value of 18 F-FDG PET/CT in patients with kidney neoplasms.Methods Seventy-nine patients (52 males,27 females,average age (57.3 ± 14.1) years),who had definitive diagnosis of kidney neoplasms by 18F-FDG PET/CT and pathological or clinical comprehensive diagnosis in recent five years,were analyzed retrospectively.The diagnosis by PET/CT was made according to the changes of kidney shape,tumor density and FDG uptake (SUVmax).The diagnostic efficacy was calculated.Results Among the 79 patients,70 cases were finally diagnosed as malignant tumors (including 40 cases of renal cell carcinoma,5 cases of renal pelvis carcinoma,8 cases of lymphoma,16 cases of metastatic tumor,and 1 case of renal fascia capsule liposarcoma) and 9 cases of benign tumors (including 7 cases of angiomyolipomas,1 case of renal acidophilic cell adenoma,1 case of metanephric adenoma; the benign tumors did not contain small lipid sample hamartoma cases).The detective rate of kidney neoplasms with 18F-FDG PET/CT was 97.5% (77/79).For the identification of benign and malignant lesions,the sensitivity of 18F-FDG PET/CT was 92.9% (65/70),specificity was 7/9,accuracy was 91.1% (72/79),positive predictive value was 97.0% (65/67),and negative predictive value was 58.3 % (7/12).Conclusions 18 F-FDG PET/CT can detect and identify most of kidney tumors.Whole-body checking and comprehensive evaluation on kidney cancer patients are still the main advantages of PET/CT.

ABSTRACT

Objective To explore baccalaureate nursing students' perception of caring, so as to provide evidence for promoting their caring behaviors. Methods Phenomenology was the chosen methodology, 11 baccalaureate nursing students were interviewed in depth. Results Themes of caring were formed into four categories: caring behavior was reciprocal; the motives of caring included fear, concern, responsibility and obligation; knowledge, technical competency and professional helping relationship constituted caring; the confusions of caring came from many factors in clinical practice. Conclusions Baccalaureate nursing students can understand the connotation of caring accurately. To encourage their caring behaviors,clinical supervisors and managers should take all kinds of measures to maintain their faith in caring.

ABSTRACT

Baculoviruses produce two viral phenotypes, the budded virus (BV) and the occlusion-derived virus (ODV). ODVs are released from occlusion bodies in the midgut where they initiate a primary infection. Due to the lack of an in vitro system, the molecular mechanism of ODV infection is still unclear. Here we present data demonstrating that Helicoverpa armigera nucleopolyhedrovirus (HearNPV) ODV infected cultured Hz-AM1 cells in a pH dependent manner. The optimal pH for ODV infection was 8.5, which is same to that in the microvilli of midgut epithelial cells, the ODV native infection sites. Antibodies neutralization analysis indicated that four HearNPV oral infection essential genes p74, pif-1, pif-2 and pif-3 are also essential for HearNPV ODV infection in vitro. Thus, HearNPV-HzAM1 system can be used to analyze the mechanism of ODV entry.

ABSTRACT

Fibroblast growth factor (FGF) is a key regulator of developmental processes. A FGF homolog (vFGF) is found in all lepidopteran baculoviruses. Autographa californica nucleopolyhedrovirus (AcMNPV) and Bombyx mori NPV (BmNPV) vFGFs are chemotactic factors. Here we analyzed the vfgf of Helicoverpa armigera NPV (HearNPV), a group Ⅱ NPV. The HearNPV vfgftranscripts were detected from 18 to 96 h post-infection (hpi) of Hz-AMI cells with HearNPV and encoded a 36 kDa protein, which was secreted into the culture medium. HearNPV vFGF had strong affinity to heparin, a property important for FGF signaling via an FGF receptor. Unlike its AcMNPV homolog, HearNPV vFGF specially chemoattracted Hz-AM 1, but not other insect cells such as Sf9 and Se-UCR and not the mammalian cells 293 and HepG2. HearNPV vFGF is also associated with the envelope of BV but is absent in occlusion-derived virus, which coordinated to the chemotatic activity analysis.

ABSTRACT

Studies have shown that transgenic plants expressing antiapoptotic genes from baculovirus and animals increase resistance to biotic and abiotic stress. However, the mechanism under these resistances is conjectural, or in some cases even controversy. In the present study, the p35 gene from baculovirus Autographa californica multicapsid nucleopolyhedrovirus (AcMNPV) was expressed in tobacco, and for the first time P35 protein was detected in transgenic plants by Western blotting. Inoculation of T1 transgenic tobacco leaves with tobacco mosaic virus (TMV) showed enhanced resistance, and DNA laddering was observed after TMV infection in control but not in transgenic plants. DAB staining showed that TMV infection did not affect peroxide induction of transgenic plants, Western blotting analysis of PR1 protein also showed no difference of control and transgenic plants. Inoculation of fungus (Sclerotinia sclerotiorum) using a detached leaf assay showed enhanced resistance of transgenic leave tissue. RT-PCR analysis demonstrated that p35 gene expression induced earlier expression of PR1 gene after S. sclerotiorum infection. Taken together, our results suggest that the mechanism under enhanced disease resistance by P35 protein is possibly related to the activation of PR-related proteins in addition to the inhibition of programmed cell death, depending on the pathogens challenged.

ABSTRACT

Objective To investigate the effects of the therapy of invigorating the kidney and qi and dissipating blood stasis to removing the toxin on the hepatocyte apoptosis and associated genes in the liver fibrosis rats,and to explore the treatment mechanism of liver fibrosis with TCM.Methods liver fibrosis model rats were induced by CCl4,and the Chinese herbs(Ganlike)with invigorating the kidney and qi and dissipating blood stasis to removing the toxin was given by daily gavage,after the experiment,the pathological changes of liver were observed with optical microscope.Hepatocyte apoptosis was detected with TUNEL assay,apoptosis-associated genes were detected with immunohistochemistry assay.Results The Chinese herbs Ganlike could obvously improve the pathological changes,alleviate the hepatocyte apoptosis and modulate the expression of apoptosis-associated genes.Conclusions The anti-fibrosis mechanism of Ganlike may be to regulate the hepatocyte apoptosis of liver fibrosis rats and to restrain the expressions of hepatocyte Fas/FasL,Bcl-2/Bax.

ABSTRACT

Objective To observe the effect of nursing intervention on type 2 diabetes in communities. Methods During Jul. 2007 to Jul. 2008, 256 cases with type 2 diabetes were chosen and intervened with non-medication treatment, direction for rational application of medication and management of the follow-up by community nurses. Results The awareness rates about blood presure, blood glucose, blood lipids and diabetes prevention & treatment among the patients were 47% , 43.8% , 60.7% and 91.5% . The compliance rates for medication, the treatment of non-medication and the attainment rate of self monitoring were 89%, 77.6% and 68%, respectively, and all of these results were significantly increased. Conclusion Nursing intervention on patients with Type 2 Diabetes in communities promotes the comprehensive treatment of diabetes.

ABSTRACT

In order to construct recombinant baculovirus carrying Schistosoma japonicum 26 ku glutathione S-transferase gene (Sj26), and observe the expression of Sj26 in mammalian cells, the Sj26 gene was amplified with plasmid pGEX-3X as template by PCR, and then recombined into T vector for sequencing. Sj26 gene was inserted into the downstream of CMV promoter of donor plasmid pFBDGC, and the recombinant donor plasmid pFBDGC-Sj26 transformed into DH10Bac, then the recombinant bacmid AcCMVSj26 was isolated and transfected into Sf9 cells. The recombinant baculovirus was harvested and final titer of vAcCMVSj26 was measured. BHK cells were transducted with recombinant baculovirus in vitro. By using Western blot, the expression of 26 ku glutathione S-transferase (GST) was detected. The results showed that after enzyme digestion and sequencing, the donor plasmid was successfully constructed. PCR confirmed that pFBDGC-Sj26 and Bacmid homologous recombination occurred in E. coli. After transfection of Sf9 cells with recombinant Bacmid, recombinant baculovirus was replicated in Sf9 cells and expressed green fluorescent protein. PCR further revealed recombinant baculovirus contained Sj26. The titer of the harvested baculovirus was 1.24 x 10(8). Western blot demonstrated that recombinant baculovirus could express 26 ku GST in BHK cells. It was concluded that Sj26 recombinant baculovirus was successfully constructed, and the 26 ku GST was expressed in mammalian cells.

OBJECTIVE:To analyze the causes of drug quality events took place in our hospital so as to provide reference for the improvement of drug quality.METHODS:Systematic investigations were performed on 150 batches of drugs that had shown quality problems during 2005 and 2006 in our hospital and the possible reasons accountable for the problems were analyzed.RESULTS & CONCLUSIONS:The quality problems were originated from 3 links including production,distribution and use of drugs,and manifested as improper or wrong packaging,labels or package inserts,inappropriate handling,and improper operation of medical staff etc,which worth the highest alert of the concerned departments.

ABSTRACT

AIM To investigate the antiplatelet effects of phospholipase C (PLC) by studying the effects of PLC on platelet adhesion and aggregation. METHODS Auxiliary agent(-), aspirin (+) and six doses of PLC were administered to anaesthetized rabbit via duodenum. Blood was taken from artery carotis before administration and at 1,2,4 hours after administration. Platelet aggregation rates were determined by turbidmetry. AIM To investigate the antiplatelet effect of PLC by de termining the concentration of TXB 2, 6-keto-PGF 1? and bleeding time( BT). METHODS TXB 2 and 6-keto-PGF 1? were detected by ra dioimmunity kit. Bleeding time were measured by routine methods. RESULTS Six doses of PLC can prolong BT significantly(P0 05).
